What is the Patient Portal? The Patient Portal is a private website that Orthopedic Associates uses to communicate with our patients; and allows our patients to securely view their healthcare summary quickly and securely. Once registered you will have the ability to send and receive secure communications with your doctor’s medical assistant, update your personal information, and view summary. Your visit summary includes allergies, assessments/plan, health concerns, medications, problems, social history and vitals.
To set up your Patient Portal access:
- Create a username, your email address by default
- Connect your Account – enter your invite code 4-digit year, ex. 19XX)
- Gain Access and message provider, request refills and appointments, review and export medical record

All patient forms must be completed using your legal name. Forms can be submitted electronically, via fax, printed or completed onsite. Forms submitted electronically through our portal are HIPAA compliant, or you may print your document to hand carry or submit by fax. Please arrive 30 minutes early to complete forms onsite.
