A Note About Insurance

We are not a party to the contract between you and your insurance company — only your insurance company, or your employer’s Human Resources department, can answer questions regarding your benefits. Please be aware that some services provided may be “noncovered”, or may not be considered reasonable and necessary by your insurance company. We suggest that you review your policy in full so that you understand what services will be covered by insurance, and what you may be responsible for.

A Note About Referral Authorization

Most insurance companies require a valid referral at the time of service – that means at your very first appointment. It is your responsibility to obtain this before you come in. Do you have a valid authorization number from your primary care physician?
If you do not have a valid authorization number, your insurance company will not pay for your visit. Referrals are not given retroactively. Because of this, we must comply with the following insurance requirements.
Your referral must be brought to your first visit. Only you can obtain a referral from a primary care physician (PCP). We cannot bill your insurance company if you do not have a valid referral at the time of your visit, and will have to reschedule your visit.
Verbal referrals are not accepted; a hard copy is required. Your PCP can fax it to 303-321-8814. Patients without a referral or insurance may be seen on a fee-for-service basis, with payment due at the visit. Co-payments must be paid at service.
