Benefits and Eligibility
We know understanding healthcare coverage can be challenging. Most Medicare replacement and HMO insurance plans require a referral to see a specialist. Contact your Primary Care Provider to assist you in obtaining the proper referral to avoid delay and to ensure your visit and any procedures or surgical care will be covered.
We also recommend that you call the toll free customer service number on your insurance card to ensure that the physicians at OCC are in-network or are preferred providers under your specific healthcare plan, or that you have out-of-network benefits. Although we will make every effort to verify your coverage prior to your appointment, we cannot guarantee coverage of services by your insurance company.
We are not a party to the contract between you and your insurance company — only your insurance company, or your employer’s Human Resources department, can answer questions regarding your benefits. Please be aware that some services provided may be “noncovered”, or may not be considered reasonable and necessary by your insurance company. We suggest that you review your policy in full so that you understand what services will be covered by insurance, and what you may be responsible for.
Please note: You are ultimately responsible for payment for all services that are not covered by your insurance plan.

Self-Pay Patients
If you are a self-pay patient (if you plan to pay without the use of insurance) or if you do not have your insurance card at the time of registration, we will require a minimum fee of $200.00 towards your anticipated services. Additional fees may be assessed at the time of check-out.
We accept all major credit cards, checks, and money orders.
Your insurance is your responsibility
Your insurance is your responsibility. To help us help you, please do the following:
Make sure to bring all of your insurance information, including your identification card. We cannot bill your insurance without this information.
Your insurance company requires you to obtain a referral to see one of our Doctors.
It is your responsibility to call your insurance company to find out what their requirements are, and have your Primary Care Physician provide a valid referral authorization number.
Be prepared to pay your Co-Pay, as required by your insurance company, at the time of service.
A Note About Referral Authorization
Most insurance companies require a valid referral at the time of service – that means at your very first appointment. It is your responsibility to obtain this before you come in.
Do you have a valid authorization number from your primary care physician?
If you do not have a valid authorization number, your insurance company will not pay for your visit. Referrals are not given retroactively. Because of this, we must comply with the following insurance requirements.
Your referral must be brought to your first visit.
Only you can obtain a referral from a primary care physician (PCP).
We cannot bill your insurance company if you do not have a valid referral at the time of your visit, and will have to reschedule your visit.
Verbal referrals are not accepted; a hard copy is required. Your PCP can fax it to 303-321-8814. Patients without a referral or insurance may be seen on a fee-for-service basis, with payment due at the visit. Co-payments must be paid at service.
